Our Mission

Real Social Care Solutions Ltd is a registered company providing services to the health and social care sectors to younger adults aged between 18 and 65 years. This includes those with complex health needs, the presentation of behaviours that challenge services, mobility needs and physical disabilities; sensory impairment (including acquired brain injury); cognitive impairment; dementia, learning disabilities and/or autism.
  • Our vision is to become the number one choice when it comes to supported living and outreach services in the East Midlands area and to one of the leading supported living and outreach service providers in England within the next 10 years.
  • We endeavour to provide a unique service that is tailor-made to meet citizens’ specific needs. We will achieve this by adopting a strength-based and person-centred approach

Tailor Made services

The principle of well-being as defined within the Care Act 2014 is the driving force behind everything we do. We recognise that everyone’s needs are different and personal to them and assume that the citizen is best placed to judge their own well-being. Delivering a tailored person-centred service is important to us and the citizen. Our combined professional experiences gained from working in health and social care give us a robust understanding of the dynamics and complexities of providing care services.

Real Social Care Solutions Ltd is committed to ensuring that services continue to meet the user’s needs.
Our Quality Assurance starts with the recruitment of the best care givers possible and continues with systematic and ongoing monitoring of their performance. Delivering a consistent high-quality service is our priority.
The service will be audited and evaluated against the National Minimum Standards, and against our service outcomes. The Service outcomes are:
  • Enhancing quality of life for citizen with care and support needs including people with long-term conditions to enable citizen to retain their independence, identity and sense of value.
  • Ensuring that citizen have a positive experience of care and support including end of life care.
  • Helping citizen to recover from episodes of ill-health or following injury.
  • Treating and caring for citizen in a safe environment and protecting them from avoidable harm.
